Por primera vez, los ortodoncistas son capaces de controlar de manera confiable el anclaje y los movimientos dentales en las tres dimensiones. En consecuencia, los miniimplantes ortodóncicos han comenzado a cambiar la práctica clínica alrededor del mundo y su uso es ampliamente considerado como el símbolo de un cambio de paradigma en la ortodoncia clínica. Proporcionan un anclaje independiente, estable y libre de cooperación en los casos rutinarios, y para los casos complejos su uso amplía significativamente la gama de problemas que pueden tratarse eficazmente con la ortodoncia. Por otra parte, las observaciones de los nuevos efectos clínicos han comenzado a confirmar o modificar las perspectivas establecidas sobre biomecánica en ortodoncia y remodelado dentoalveolar.
En concreto, el miniimplante, y no el aparato extraoral, representa ahora el estándar oro para el anclaje. Igualmente, muchos ortodoncistas han encontrado que los miniimplantes son mínimamente invasivos, seguros, fiables, bien tolerados, rentables y de gran utilidad en múltiples escenarios clínicos.
Sin embargo, mi propia experiencia clínica y numerosos aportes detallados a la literatura mundial (resumida en el Capítulo Dos) me han convencido desde entonces de que los miniimplantes producirán cambios fundamentales e irreversibles en la práctica ortodóncica en términos de reforzamiento del anclaje rutinario y aplicaciones clínicas de gran alcance, incluso versátiles.
Este libro tiene como objetivo proporcionar al ortodoncista la información teórica y clínica esencial sobre miniimplantes para que puedan introducir fácilmente el anclaje esquelético en su práctica clínica en una gran variedad de escenarios clínicos comunes (en especial, conjuntamente con los aparatos de alambre recto).
Sin embargo, la experiencia de la provisión de cursos de miniimplantes me ha enseñado que los colegas con experiencia o confianza limitada con los mismos tienen una necesidad justificada por conocer las pautas clínicas básicas basadas en la mejor práctica y evidencia disponible. En consecuencia, este libro está estructurado de manera que los lectores puedan seguir fácilmente los principales pasos para un caso clínico específico, y aprendan al mismo tiempo las principales ventajas del miniimplante como anclaje.
